Five fields
- Setup type — breakout continuation or reversal after failure
- Level — the price that had to hold or break
- Trigger — the behaviour that allowed entry
- Invalidation — where the idea was wrong
- Process grade — did you follow the checklist, regardless of P&L
Weekly review
Once a week, sort notes by process grade. Patterns in missed confirmation or early entries become obvious faster than scanning raw profit figures.
